Job Description

Main Duties and Responsibilities

To provide a statutory Social Work Service in accordance with the provisions of relevant legislation and national and local guidelines.

To assume case management responsibilities specific to the Service, carrying out assessment of service users needs and risks, where necessary using accredited risk assessment tools. The post will necessitate working in a multi-disciplinary team with a particular focus on Intermediate Care/Reablement. The role will also involve participating in structured consultation/supervision sessions as required, and representing the Health & Social Care Partnership in multi-disciplinary forums. This will all be in accordance with the Council or hosted stakeholders arrangements, through a Service Level Agreement or Partnership agreement.

Requirements

This post is considered regulated work with children and/or protected adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. You will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by Clackmannanshire Council.
