An exciting opportunity has arisen for an enthusiastic Occupational Therapist to join the acute hospital / community rotation.
The post will involve working in an allocated rotation across various acute hospital in-patient wards as well as the community hospital in-patient wards and the community rehabilitation team. The focus is the provision of comprehensive OT assessments and interventions to help maximise client function and independence focussing predominantly on complex discharge management, prevention of admission, rehabilitation, management of frailty and the provision of equipment and adaptations. Whilst on rotation in the community, there is an expectation that the post holder will act as Lead Professional for patients on reablement.
Regular supervision is provided through peer support groups and formal supervision sessions and opportunities for ongoing professional development are encouraged.
A Degree or Diploma in Occupational Therapy is required as is current HCPC registration.
You will be expected to travel efficiently and effectively between various work locations to meet the operational requirements of the Service. A full current driving licence and use of own car for work purposes is essential.
