Post Title: Paediatric Rehabilitation Co-ordinator
Hours: 12.5 Hours per week
Salary: £13,624 - £15,948 p.a.
Salary placing will normally be at the first point of the scale.

Job Purpose: To co-ordinate the rehabilitation of all Paediatric major trauma patients, acting as a key contact to ensure timely and seamless repatriation of patients from the Major Trauma Centres (MTC), and support transitions between services/operational areas across the Highland Health and Social Care Partnership.
To initiate and assist with rehabilitation assessments of patients admitted with major trauma and/or complex rehabilitation needs, and support the development and sign off rehabilitation plans for this patient group. The postholder will ensure
- Goals are communicated to patients, their families and to all the acute and rehabilitation team members.
- Progress is reviewed, along with the wider trauma team, to determine the best timing for transfer to the most appropriate rehabilitation facility for further treatment.
Act as a senior coordinator regarding team assessments across a range of disciplines, within the trauma team, and alongside Consultants in Rehabilitation Medicine.
Ensure that the patients and their families are informed and educated about the concept of rehabilitation after trauma and are aware of the treatment processes, as well as of the long term management plans once back in their communities.
Co-ordinate training and education for staff, patients and carers across NHS Highland, and Highland Council.

Degree level education or equivalent professional qualification in nursing or allied health professional.
Registered with NMC or HCPC as relevant.
Postgraduate education to Master’s degree level (or have an equivalent level of knowledge gained through experience, with postgraduate or other relevant qualifications desirable).
