A full time Community Care Worker (CCW) post has arisen in the Duty Response and Rehabilitation Team
CCWs play a crucial role in this front line service. Working alongside Social Workers the team is the first point of response for all referrals to Adult Social Work. The work includes Hospital Discharge, Crisis Response and a range of other interventions designed to provide a prompt response to service users.
CCWs work with people across the full range of community care client groups and require a broad range of skills which cover the range of issues that people present with (both Social Work and Occupational Therapy type work). Examples include assessing crisis situations, arranging services to support crisis, follow up after hospital discharge, falls screening and assessment, minor changes to support packages, facilitating supports to avoid unnecessary waits, good working knowledge of a wide range of community supports.
Within defined boundaries, co-ordinate the provision of a range of services and equipment to meet the assessed needs of individuals requiring assistance, to enable them to maintain and maximise their independence, prevent admission to hospital or a care home, facilitate hospital discharge and to make choices.

Educated to HNC or SQA Level 3 in social care or equivalent and be able to
demonstrate equivalent knowledge, skills and competencies gained through relevant experience.
As the duties of this role require you to effectively travel to successfully undertake the full remit of the role, you must hold a current driving licence and have access to a vehicle.*
