Support Worker for Unpaid Carers – Mid Argyll

18 Hours

Responsibilities

  • Visit Unpaid Carers at their home or another suitable location. Support the Unpaid Carer in accordance with their Adult Carer Support Plan (ACSP)
  • Work with the Mid Argyll Assessment Support Worker to regularly review Unpaid Carers’ ACSP
  • Support Unpaid Carers to maximise benefits and entitlements and complete forms where necessary and appropriate
  • Support and advocate on behalf of Unpaid Carers, at meetings with Social Work and other agencies
  • Assist with organising and hosting Dochas support / social groups, including lunches, condition specific, e.g., Dementia, Parkinson and Autism helping with the inviting of relevant speakers and guests to promote Unpaid Carers wellbeing and knowledge
  • Make appropriate referrals to other agencies
  • Deliver services in accordance with centre policies and procedures and in compliance with the service level agreements of the funding agencies
  • Promote and raise Unpaid Carers’ awareness of their legal entitlement to have an assessment made of their own needs
  • Encourage Unpaid Carers to notify their GP of their caring role
  • Ensure appropriate mechanisms are maintained and developed to:

a.     Provide information to Unpaid Carers

b.     Give Unpaid Carers a voice on areas affecting them

Organisational Development

  • Ensure that accurate information and statistical records are maintained. Produce and submit accurate, timely and appropriate reports to the required standard to ensure achievement of annual outcomes and associated targets
  • Promote and represent Dochas with partner agencies and at relevant meetings, forums and events, as directed by the Manager
  • Attend team meetings on a regular basis
  • Undertake personal development and training as required in line with operational responsibilities
  • Contribute to the production of newsletters/annual reports of Dochas Centre as required

    Management – in liaison with Centre Manager
  • Working with the Mid Argyll Assessment Support Worker,
  •  

a.     Facilitate the process to match staff with Unpaid Carers for Time4Me

b.     Induct, support and supervise Time4Me staff

c.      Regularly review Time4Me cases

Quality Assurance and performance management

  • Participate in regular monitoring and evaluation of all services to improve the quality of the services provided

Other duties

  • To ensure at all times fully comply with equal opportunities policies in all aspects of human resource activity and in the provision of services, within the line-management
  • To undertake any other duties commensurate with the post-holder’s responsibility.
