18.5 hours per week, Shift working over seven days
£39,741 - £43,561 pro rata / £20.60 - £22.58 per hour (including shift allowance and Distant Islands Allowance)

Orkney Health and Care is offering an opportunity for a highly motivated, pro-active individual to join our established care at home team in the role of part time Social Care Co-ordinator. This exciting role will offer the successful candidate an opportunity to work closely with a range of professionals in the efficient and effective delivery of care at home services, focusing on a reablement. The post holder will work with a range of multi-professional teams supporting short term reablement focussed packages of care.

Using a short term reablement approach means that individuals are assessed at home, via a multi-disciplinary team, with care at home services being tailored to support those individuals achieve their outcomes and goals and maintain their personal independence. Therefore, the successful candidate for this post will have knowledge and understanding of the roles of other agencies in relation to joint working.

Social Care Co-ordinators work in a dynamic and fast paced environment, leading and co-ordinating the day-to-day operations of a high-quality care at home service. This means management of care at home staff based across geographical localities of Orkney. Therefore, the ability to demonstrate skills in workload management and prioritisation are essential. The successful candidate will also be able to demonstrate experience of previous leadership and supervisory roles and of working with individuals in a social care service. Orkney Health and Care take great pride in the community services they deliver so it is important that the successful candidate has experience of working with and an understanding of the Health and Social Care Standards, current relevant Legislation, Codes of Practice and the knowledge and experience of an approach to work consistent with Human Rights, Equal Opportunities and Anti-discriminatory practice.

Other essential requirements of the Social Care Co-ordinator’s role is being able to demonstrate:
• Experience of being responsible for Health and Safety including Risk Assessments;
• Assessment and support planning experience.
• Involvement previously in change programmes and the ability to communicate the vision of the service.

This post is subject to membership of the Protecting Vulnerable Groups (PVG) Scheme in respect of regulated work with adults.

You should be able to travel efficiently and effectively between work locations within Orkney to meet the operational requirements of the Service.
