Job Description

We are currently looking for a Home Support Care Co-ordinator to join our busy, demanding and developing Reablement and TEC team. You will work directly to the Team Leader and will be an effective communicator and team player with experience of managing and motivating a team of support worker working in a Care at Home Service. You will work as part of the Clacks and Stirling Health and Social Care Partnership. You will be responsible for ensuring the day-to-day management and supervision of a team of support workers within a team within the service area. Ideally, you will have experience of working in a care at home environment or working in the community. You will be required to be flexible, responsive able to prioritise workloads, monitor and review performance and work within the National Health and Social Care Standards. You will have a good understanding on change management and have a creative approach to service delivery. A good understanding of Reablement approach and Technology Enable Care is required. The post is demanding, continuous change, diverse and requires you to be able to work with a number of different teams and partners both internal and external. You will be required to register with the SSSC.
