Additional benefits include flexible working hours and practices including hybrid working, generous annual leave entitlement, employee benefits scheme (including car leasing), and pension scheme.

As a member of a generic locality team integrated within the Health and Social Care Partnership, you will work as a partner in the Local Integrated Care Service (LINCs), and alongside people, families, and communities. The ideal candidate will be motivated and engaging, and possess excellent interpersonal and communication skills.

Effective time management and organisational skills are essential for an autonomous worker to successfully manage a complex caseload.

The ability to be flexible and respond to crisis / competing demands are essential to the role.

An in-depth understanding of Self Directed Support and how this can be successfully delivered in a sometimes challenging and rural context, is essential.

The ability to integrate and cultivate successful links within the team, partnership as a whole, across key services, and with the community, is highly desirable.

Knowledge of services, challenges, demands, and demographics within PKC would be an advantage.

An ability and commitment to work creatively and constructively to achieve truly personal outcomes together with your clients is critical.

You must have experience and skills of undertaking person centred assessments and reviews.

You will be required to demonstrate a sound knowledge and understanding of social work principles and values.

Experience and knowledge of Adult Support and Protection Procedures and all relevant legislation are essential.

You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.

The Individual

The successful candidate will assist in maintaining vulnerable people in the community, by providing a range of social work and care management services in accordance with the Health and Social Care Standards and the SSSC Codes of Practice.

This post may be suitable for candidates who have recently completed their Social Work Degree but have not yet graduated.  In this circumstance, a Social Work Assistant contract will be offered until you receive your qualifying certificate and are eligible to register with the SSSC as a Social Worker.
