An exciting opportunity has arisen for a dynamic Advanced Practitioner in Occupational Therapy to lead the implementation in home is best model of care. You will be based within Nairn Town and County Hospital and provide services to Nairn and the wider Nairnshire population.
This is a service implementing the home is best model of care. In doing this, you will be vital in promoting flow from acute services and reducing admissions. You may also pass work streams to the mainstream AHP team based in Nairn.
As the successful post holder, you will be expert in frailty and demonstrate this by managing a diverse range of conditions. You will work individually and as part of an integrated model of care, with multidisciplinary and multi agency team members. You will deliver the best standard of care to a fast paced and highly complex caseload and you will use evidence based practice and client centred principles to plan and execute tasks. You will manage your service delivery for home is best, ensure flow from various work streams and delegate duties to Senior Enablement development workers within this service. You will be part of a team that works closely alongside existing mainstream services, other Team Leads and the Integrated Team Manager.
In addition, you may use your advanced clinical ability to provide onsite mentorship to mainstream Occupational Therapy colleagues based within Nairn Town and County.
In order to achieve the above, you will have full professional support from the AHP Team Lead and the Integrated Team Manager.
Applications are invited from experienced Occupational Therapists who can demonstrate a high level of clinical expertise within frailty which your previous track record should highlight. An advantage would be an ability to demonstrate an involvement in quality improvement.
Your will receive support from the AHP department which offers formal protected learning time, including peer review and in service training programmes.
To be considered for this post you must be HCPC registered and hold a BSc or MSc degree in Occupational Therapy. In addition, you must be a UK car driver with access to a vehicle due to community working.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
