Are you a leading edge pharmacist ready to take on a new challenge? Do you want to be part of a new team that will re-shape primary care services? We are recruiting two additional Advanced Pharmacist Practitioners to join our existing team of Advanced Pharmacist Practitioners and Specialist Clinical Pharmacists. These posts are being created in response to Prescription for Excellence and the Scottish Government's Primary Care Investment Fund. As an Advanced Pharmacist Practitioner, you will be responsible for medicines management at a GP practice in order to optimise the pharmaceutical care of patients. This will include: overseeing all repeat prescribing, providing all annual medication reviews and polypharmacy reviews, undertaking all medicines reconciliation, and delivering intensive medicines management for a caseload of patients. For all of these functions, you will take clinical responsibility for patients' medicines management. In addition, you will provide expert advice to the multidisciplinary team within the practice to ensure the safe and effective use of medicines across all clinical conditions. We are recruiting to two posts, one based in North Highland (Caithness) and one based in West Highland (Lochaber). The North Highland post will involve working at a number of different GP practices, with the potential for some involvement within the local hospital service. These posts require excellent therapeutic knowledge (demonstrated through a post-graduate qualification) and an independent prescribing qualification. North & West Highland Operational Unit supports the development of health professionals to deliver a high quality service, therefore if you do not posses these qualifications we would support you to achieve them within the first year of post (courses start January 2017). You must be able to demonstrate a commitment to continuing professional development, be highly organised and have good IT skills. North and West Highland is a beautiful part of Scotland which offers an excellent quality of life in a rural setting.
