Assessment, care planning, monitoring and reviewing of care plans for a case load of service users with disabilities/mental health conditions including case recording. Preparing reports, attending meetings as required for the benefit of service users within this case load. These tasks will be carried out in partnership with service users, family members, social work colleagues and other professionals, agencies as necessary to meet need.
Fife Council pays the Scottish Local Government Living Wage Rate to employees earning less than £8.33 per hour. If the hourly rate of pay for the post is less than the Scottish Local Government Living Wage Rate a supplement will be paid.Requirements
You must have the ability to travel throughout Fife.
There will be a requirement to work within a mobile and flexible workforce as well as working outwith normal working hours and over public holidays, as necessary, to meet the needs of the service.
Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.Responsibilities
You will carry out professional assessments and the management of care plans as directed by the Service.
You must provide professional advice, guidance and support to adults with additional support needs on a case work basis.
You will also actively work with adult services users and carers in order to take into account their views and needs.The Individual
A Diploma in Social Work (or equivalent) qualification is essential, as is registration with the Scottish Social Services Council.
To practise as a Social Worker it is essential that the applicant holds a Diploma in Social Work. Consideration will, however, be given to applicants who have completed the Diploma but have not yet received confirmation of qualification. In this case you will be appointed and paid as a Social Work Assistant until the Social Worker appointment can be confirmed.
Experience of working within a team is essential as is experience of working within a Social Work setting.
