The successful candidate will be based within a well-established team delivering a service to adults in the community who have a range of disabilities, mainly mental health problems however some physical disability and autism.
The Adults Assessment and Care management service for is for those people aged 16 to 65, who are resident in West Lothian and have both mental health and other disabilities.
This is an interesting and challenging opportunity for an enthusiastic person to work with a range of individuals who require support to maintain their independence.
The worker must have an ability to work as part of a team, respond to demands of a busy service, be able to follow procedure, have excellent communication and good report writing skills.SVQ3 and a HNC in Social Care or equivalent. Relevant experience in a social or health care setting is essential. It is desirable for candidates to have relevant mental health knowledge and assessment and care management experience in a social work practice team.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
