To provide the role of primary care-giver to older people living in a residential care setting, to the National Care Standards established by the Care Inspectorate.
KEY TASKS AND RESPONSIBILITIES
Corporate ResponsibilitiesTo undertake individual role in line with the Council's Strategic Priorities and in compliance with the Council's corporate policies and procedures. To undertake any other reasonably required duties as instructed by Management or someone acting on their behalf, in addition to the role specific tasks & responsibilities detailed below.
Role Specific Tasks & ResponsibilitiesWork as part of a team delivering care to older people living in a residential care setting. To be aware of and at all times promote the National Care Standards and SSSC codes of Practice. To carry out the role of Key worker to residents and their families, including person centred care planning and meeting the individually assessed needs and wishes of the resident. Carry out the continuous assessment of activities of daily living. Put in place effective person cantered plans and procedures, which identify the needs of the residents and address these needs in an individualised manor. Help older people adapt to change positively. Be involved in the formulation, implementation and planning of reviews for residents taking into account their changing needs, and planning appropriate care plans. Undertake extended personal care tasks as per agreed protocols. Promote service users choice, dignity and independence at all times. Undertake recording, report writing and other appropriate administrative processes. Liaise with other agencies and staff as appropriate regarding resident's needs. Undertake training and professional development. Ability to work to and achieve SVQ level 3 in Social Care Register and comply with the SSSC codes of practice and meet any requirements they may require of you.
ESSENTIAL / DESIRABLE CRITERIA
Education and QualificationsAbility to work towards and achieve SVQ level 3 in Social Care SVQ level 3 of HNC in Social Care or equivalent qualification recognised by the SSSC Application Form Interview
