 21 hrs per week, salary pro rata, + 14% shift allowance

West Lothian Care Homes for Older People provide care needs older people with long term mental health problems and for frail older people. We are looking for people interested in working with older people in this environment. A rotational shift system is in operation, which includes weekend working.

The key duties of this post are:

To undertake responsibility, jointly with other social care staff, to provide care and support to all residents which respects their individual rights and choice. To act as “key worker” for a number of individuals including: Pre-admission visit(s) to the client at home or current residence Preparation of individual plans of care Monitoring of plans of care: ensuring changing personal care needs are met Delivering direct personal care in accordance with plans of care To report significant operation/events etc. relating to care of the resident To participate in reviews and to provide written reports as appropriate Establish and maintain therapeutic relationships Assess and plan nursing care according to the needs of the resident using individual plans of care Maintaining residents’ personal files by details recorded on a daily basis in accordance with the “Open Access” Guidelines To work in collaboration with families/carers/other agencies to provide a service to residents To have a lead role in the delivery of social care to individuals with severe and enduring mental illness To monitor and support the work of staff, and assist the mangers in achieving and maintaining high standards of care. To foster a multi-disciplinary approach to clinical and social behavioural issues. To provide direct personal care in accordance with resident’s individual plan of care by, for example: Assist/support with personal hygiene and appearance, i.e. bathing, shaving, nail cutting, attending to laundry Assist/support with dressing/undressing Assist/support mobility Enable residents to eat and drink Enable residents to access and use toilet facilities Contribute to the management of resident continence Enable residents to achieve physical comfort Contribute to the care of the deceased person Enable residents to maintain contacts in potentially isolating situations, e.g. due to poor mobility, communication problems due to a stroke, assistance with letter writing Administration of medication in accordance with Service Guidelines to include: Accurate recording; assisting with accurate ordering and recording, and recording of delivery; monitoring of side effects; monitoring those residents who choose to self-medicate. To ensure that each individual reaches personal goals in accordance with capacity planning guidelines

The skills and experience you must have are:

Registered Mental Nurse Qualification and experience of older people with severe and enduring mental health illnesses

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.

This position requires registration with the SSSC. The successful candidate will have six months from commencement in this post to register with the SSSC.
