Western Isles Hospital, Macaulay Road, Stornoway, Isle of Lewis, HS1 2AF Department Dementia Clinical Nurse Specialist Band 6 £26,830-£35,933 plus £985 Distant Islands Allowance 37.5 hours per week Fixed term Contract – One Year An Innovative Service Aimed at Breaking Down the Barriers for Dementia Diagnosis After a successful test of service provision, NHS Western Isles will be continuing Memory clinics within primary care and expanding this to cover all the islands with a clinic in each practice. The post holder will be line managed by the Nurse Consultant for Older People and Dementia within the Community Care Team and will work directly in GP practices with minimal supervision. For people with cognitive impairment, the locally-based Memory clinics will allow ease of access. The clinician will engage the person in an efficient assessment process, person-centred provision, diagnosis in a familiar setting and a trusted source of health advice with a cost-effective, structured process and also provide direct links for treatment and post-diagnostic support. The post holder will assist surgeries to meet the national standard for first appointment within 4 weeks. All assessments are carried out by the DCNS and the information entered directly onto surgery systems and there will be no external referral process. The post holder will provide expert advice on management and care of people with stress and distress in primary care, self-audit of service efficiency and improved rates of treatment interventions. The post holder will require a high level of assessment skills and expert knowledge in dementia with the ability to provide a differential diagnosis and subtype. The post holder will be expected to travel across the islands on a regular basis and it is anticipated most of the time will be spent in GP practices.
