Social Work post in Adults Mental Health team
The successful candidate will be based within a well-established team delivering a service to adults in the community who have a range of disabilities, mainly mental health problems however some physical disability, learning disability and autism.
The Adults Assessment and Care management servicer is for people aged 16 to 65, who are resident in West Lothian.
This is an interesting and challenging opportunity for an enthusiastic person to work with a range of individuals who require support to maintain their independence.
The worker must have an ability to work as part of a team, respond to demands of a busy service, be able to follow procedure, have excellent communication and good report writing skills.
These tasks will be carried out in partnership with service users, family members, social work colleagues and other professionals and agencies as necessary to meet need. You will be required to have a good understanding of Self Directed Support and completing needs led and outcome focused assessments.
The worker will also undertake ASP investigations in accordance with service standards.
You must have the ability to travel throughout West Lothian and hold a recognised qualification in social work and be registered as a social worker with SSSC.
Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.
The skills and knowledge you must have: -BA in Social Work/QSW or equivalent.
