We are looking for an experienced social worker who has a particular interest in the assessment and review of people supported through Self Directed Support which reflects the common goals of Health and Social Care policy to deliver better outcomes for individuals. The post will be based in the Care at Home Review Team and the successful candidate will have the opportunity to work with supported people and providers in the planning and review of outcome focused support which requires creativity and innovative thinking to ensure that all resources which are available are recognised.Job Details
